Online Registration and Participation Forms

Please download the Roster & Waiver Form and have each parent/guardian sign this form and bring it to the check-in desk 60 minutes prior to your first scheduled game. This form is required and must be completed prior to the start of the tournament. No player will be allowed to play without the signature consent of the parent or guardian.
Click here to Register Your Team online. Payment is not require to register your team online. Your team registration form you fill out will be emailed back you. This form must be included with your payment by check made payable to: Kona Crush Soccer Academy, PO Box 1481, Kailua-Kona 96745.
Early Bird Discount is $180 (on or before December 15, 2017), Standard Fee is $200 (before December 31, 2017) and Late Fee is $225 for up to 6 players. Tournament entry deadline is January 8th, 2018.
To receive Early Bird Discount, please have payment and team registered by December 15, 2017. Late fee registration on or after January 1, 2018. Please review the RULES page before registering.
Your team will not be added to the schedule until the full registration fee is received.
Also, please note KCSA does NOT take individual player payments. The team coordinator is responsible for gathering player payments and submitting the team registration fee to KCSA.
Paper Registrations not arriving in the US Mail or not received KCSA on or by January 9th, 2018 subject to space availability in an appropriate division.
